Social Work Case Manager

At LifeBridge Health, we are seeking a skilled Social Work Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring efficient care for our inpatients, minimizing delays and denials whenever possible.
Key Responsibilities:
- Partner with healthcare teams to coordinate patient care
- Lead role in coordinating with community agencies to address patient needs
- Link patients to financial resources and other support services
Requirements:
- Master's degree in Social Work required
- 2-4 years of Social Work experience required
- Recent experience in Medical Social Work preferred
- LCSW-C - Certified Clinical Social Worker required
