At LifeBridge Health, we are seeking a skilled Social Work Case Manager to join our team. As a key member of our healthcare team, you will play a vital role in ensuring efficient care for our inpatients, minimizing delays and denials whenever possible.
Key Responsibilities:
- Partner with the healthcare team to coordinate patient care and services.
- Lead the coordination with community agencies to address abuse, neglect, substance abuse, mental health, or adoption.
- Link patients to financial resources and other support services.
Requirements:
- Master's degree in Social Work required.
- 2-4 years of Social Work experience required.
- Recent Social Work experience in Medical Social Work is preferred.
- LCSW-C - Certified Clinical Social Worker required.
