About the Role:

The Family Nurse Practitioner will be responsible for providing high-quality patient care in our Residential Treatment Programs in Fairfield and Seymour, CT. This is an excellent opportunity for a Nurse Practitioner to make a meaningful difference in the lives of adolescents and young adults.

Key Responsibilities:

  • Provide direct patient care, including evaluation, assessment, medication management, and psychotherapy.
  • Develop and implement individualized treatment plans in collaboration with the multidisciplinary team.
  • Conduct weekly follow-up appointments and ensure timely completion of documentation.
  • Collaborate with other healthcare professionals to ensure continuity of care.
  • Participate in required treatment teams, staff meetings, and training.

Requirements:

  • Master's Degree in nursing from an accredited institution.
  • Licensed as a Nurse Practitioner in the state of Connecticut.
  • Current DEA license.
  • Minimum two years of experience working with adolescents or young adults.
