Nurse Practitioner or Physician Assistant

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, reviewing their current health, medical history, medication adherence, social environment, and other risks.
This provides unparalleled insight into patients' overall health and well-being, which can be challenging to capture during routine office visits. By working with our team, you will have the opportunity to make a meaningful impact on patients' lives and contribute to improving the quality of care.
Responsibilities- Conduct home visits to assess patients' health and well-being
- Review patients' medical history, medication adherence, and social environment
- Identify potential health risks and develop plans to mitigate them
- Collaborate with healthcare teams to ensure seamless care coordination
- Document patient information and assessments accurately and thoroughly
- Masters degree in Nursing or Physician Assistant studies
- Current RN and NP/PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong communication and interpersonal skills
- Ability to travel to patients' homes and work in a variety of settings
