About the Role
In this role, you will be responsible for coordinating care services for eligible individuals, including outreach, enrollment, and retention. You will work closely with our Care Coordination team to identify and engage members who require Health Home services. Your goal will be to ensure that all necessary resources are available to support their well-being and independence. To succeed in this position, you should possess a strong understanding of Medicaid services and policies, as well as excellent communication and interpersonal skills.

Key Responsibilities
  • Develop and implement effective care plans to address the unique needs of each client.
  • Collaborate with Care Coordinators to identify and prioritize members' needs and develop strategies to meet those needs.
  • Establish and maintain relationships with community resources and healthcare providers to ensure seamless coordination of services.
  • Document all interactions and outcomes in our electronic health record system.
  • Participate in ongoing training and professional development to stay up-to-date on industry best practices and regulations.
Estimated salary: $45,000 per year.
