MedStar Health is seeking a skilled Social Worker to provide care coordination, psychosocial support, and community outreach to a panel of frail elders receiving home-based primary care. The ideal candidate will have demonstrated high-level problem-solving, communication, and independent judgment skills.

The Social Worker will be a member of an interdisciplinary team responsible for ensuring outcomes aligned with the goals of care for their panel of patients. They will provide comprehensive intake assessments, develop goals of care, and communicate with service providers and city/state agencies.

Primary Duties:

  • Document all activities, including completion of comprehensive intake assessments and development of goals of care.
  • Interview patients, family members, and caregivers to evaluate medical, psychosocial, financial, and environmental needs.
  • Formulate accurate assessments, intervention plans, and assist with enrollment in eligible programs.
  • Maximize patient, family member, and caregiver ability to comply with medical recommendations and goals of care.
  • Participate in clinical competency review meetings to improve quality of care.

Qualifications:

  • Master's degree in social work.
  • 1-2 years' experience.
  • LGSW in proper jurisdiction.
  • American Heart Association (AHA) or Red Cross CPR certification.
