Job Summary:

The GEO Group, Inc. is seeking a highly skilled Certified Social Services Professional - Rehabilitation and Reentry Specialist to join our team. This role provides an array of reentry and case management related services to participants located within a corrections-setting.

Job Description:

As a Certified Social Services Professional - Rehabilitation and Reentry Specialist, you will operate in partnership with the department of corrections within a multivendor treatment model. Your primary duties and responsibilities will include:

  • Providing complex case management services.
  • Coordinating discharge planning activities.
  • Reviewing case information, conducting assessments, providing diagnoses (under the supervision of an LCSW-C), and treatment recommendations.
  • Assisting participants with navigating community resources and services as they reestablish their lives within communities.
  • Identifying reentry and integration barriers and providing support and assistance in overcoming those barriers.
  • Developing, monitoring, and modifying treatment plans in collaboration with the multi-vendor team.
  • Serving as a liaison between participants and community resources to ensure efficient processing of referrals.
  • Assisting participants with obtaining social services and other healthcare benefits.
  • Providing psychoeducation and counseling as it relates to preparation for reentry/transition.
  • Conducting individual and group counseling (with a focus on substance use treatment).
  • Consulting with colleagues and other DPSCS and GEO staff related to reentry/transition barriers or challenges.
  • Participating in professional meetings, committees, trainings, and conferences.
  • Participating in research and Quality Assurance and Improvement (QA & I).
  • Performing other duties as assigned.
Requirements:
  • Masters degree program from an accredited school of social work.
  • Valid/current license as a Licensed Master Social Worker (LMSW) as issued by the state in which the work is performed.
  • Knowledge of the principles, procedures, techniques, trends, and literature of social work with particular reference to clinical social work; psychological or social aspects of mental and developmental and physical disabilities.
  • Knowledge of community organization principles; scope and activities of public and private health and welfare agencies; characteristics of mental, developmental, and physical disabilities; current trends in mental health, public health, and public welfare, and Federal and State programs in these fields.
  • Ability to utilize and effectively apply the required technical knowledge.
  • Ability to establish and maintain the confidence and cooperation of persons contacted within the context of work.
  • Ability to secure accurate psycho and social data and record such data systematically.
  • Ability to prepare clear, accurate, and concise reports.
  • Ability to work with family and community agencies in preparation for discharge.
  • Ability to develop and implement programs.
  • Ability to provide professional consultation; analyze situations accurately and take effective action.
  • Ability to communicate effectively through written or verbal modes.
  • Ability to work with computers and the necessary software typically used by the department.
Compensation and Benefits:
