**Home Care Specialist Job Description**
Nuvance Health is seeking a dedicated Home Care Specialist to join our team at NHCH Caregivers at Home. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of our clients, providing personalized care and support in the comfort of their own homes.
**Key Responsibilities**
- Provide assistance with personal care activities, including bathing, oral hygiene, feeding, and dressing
- Utilize assistive devices to ensure client safety and mobility
- Assist with meal preparation/cooking and/or feeding, maintaining sanitary kitchen conditions
- Identify and report changes in client condition, providing accurate updates to caregivers and medical professionals
- Provide basic homemaking and laundry assistance, maintaining a safe and orderly environment
