Occupational Therapist

We are seeking highly skilled Occupational Therapists to join our exceptional home healthcare team at Nuvance Health.
Key Responsibilities:
- Develop and implement personalized care plans in collaboration with patients, their representatives, caregivers, and physicians.
- Deliver ordered occupational therapy services, ensuring seamless communication with all involved physicians and healthcare practitioners.
- Provide direct occupational therapy services, adhering to plans of care established by physicians.
- Maintain accurate visit records, including evaluations, care plans, client responses, and discharge planning when applicable.
- Participate in case management conferences with nurses and other team members to coordinate occupational therapy programs with overall care plans.
- Offer education and counseling to patients and family members regarding occupational therapy treatment regimens.
- Identify patient and family needs for additional home health services, making referrals as necessary.
- Contribute to the agency's quality assurance and performance improvement programs, as well as sponsored in-service training.
- Supervise care provided by Certified Occupational Therapy Assistants (COTAs) and home health aides as required.
- Regularly re-evaluate client progress and treatment regimens, making adjustments as needed under physician recommendations.
- Fulfill all compliance responsibilities, including in-service training, and maintain Nuvance Health's Foundational Values.
- Perform other duties as required.
Requirements:
- Valid driver's license and reliable vehicle access.
- Bachelor's degree in Occupational Therapy or related field.
- Current CT State Occupational Therapy License.
- Minimum one year of experience, with preference for qualified home health agency experience.
