RN Case Manager

At CenterWell Home Health, we're dedicated to delivering exceptional patient care and support. As a Home Health RN Case Manager, you'll play a vital role in helping patients recover and regain their independence in the comfort of their own homes.
About the RoleWe're seeking a skilled and compassionate RN to join our team as a Home Health RN Case Manager. In this role, you'll provide admission, case management, and follow-up skilled nursing visits for home health patients. You'll work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.
Responsibilities- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare providers
- Provide skilled nursing care and education to patients and their caregivers
- Coordinate with other healthcare professionals to ensure seamless care transitions
- Monitor patient progress and adjust care plans as needed
- Diploma, Associate, or Bachelor's Degree in Nursing
- Minimum of one year of nursing experience, preferably in home health or a related setting
- Strong med-surg, ICU, ER, or acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Strong organizational and communication skills
