RN Case Manager

We are seeking a skilled RN Case Manager to join our team at Carilion Clinic. As a key member of our healthcare team, you will be responsible for providing case management services to assigned patient populations.
Key Responsibilities:
- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with healthcare providers, social workers, and other stakeholders to ensure seamless care transitions
- Conduct thorough assessments of patient needs and develop strategies to address barriers to care
- Provide education and support to patients and families to promote optimal health outcomes
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
Requirements
- Current licensure as a Registered Nurse in Virginia
- 3+ years of experience in a clinical healthcare setting with case management responsibilities
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of community resources and healthcare systems
