About the Role

We are seeking a highly skilled Behavioral Health Care Coordinator to join our team at UnitedHealthcare. As a key member of our integrated care model, you will play a vital role in ensuring that our patients receive the best possible care.

Key Responsibilities
  • Assess and Plan Care: Conduct thorough assessments of patients' needs and develop personalized care plans to ensure they receive the most appropriate level of care.
  • Coordinate Care Services: Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Manage Care Plans: Oversee the care plan throughout the continuum of care, serving as a single point of contact for patients and their families.
  • Communicate with Stakeholders: Effectively communicate with all stakeholders, including patients, families, healthcare providers, and community resources, to ensure quality coordinated care and services are provided.
  • Advocate for Patients: Advocate for patients and their families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.
Requirements
  • Licensure: Licensed Social Worker (LMSW, LBSW, LCSW) or current unrestricted RN license, NP, PLP, LPA, or LPC license in the state of Texas.
  • Experience: 3+ years of experience working within the behavioral health setting.
  • Skills: Intermediate level of proficiency with Microsoft Word, Excel, and Outlook, as well as reliable transportation and the ability to travel to visit patients in their homes and other settings.
Preferred Qualifications
  • Education: Bachelor's degree, preferably in a behavioral health or clinical field.
  • Experience: Medicaid Waiver experience, case management and/or care coordination experience, behavioral health experience, field-based work experience, and experience with electronic charting.
