We are seeking a highly skilled and compassionate Care Manager to join our team at STR Behavioral Health. As a key member of our collaborative care team, you will play a vital role in supporting the needs of our clients throughout their treatment process.
Key Responsibilities- Support clients throughout the treatment process, including administrative tasks and client orientation.
- Assist with the admissions process, ensuring a smooth transition for new clients.
- Provide exceptional customer service to clients, families, and clinical partners.
- Follow up with clients after treatment to ensure access to necessary resources.
- Manage issues related to FMLA, EAP, Disability, Legal, Financial, and Personal Therapy.
- Coordinate communication with clinical partners to facilitate admissions and ongoing client care.
- Facilitate group sessions and support clinical team with schedule coordination, discharge planning, and records maintenance.
- Support client communication and feedback initiatives, including satisfaction surveys and feedback mechanisms.
- Bachelor's degree with a minimum of 3+ years' experience in a behavioral healthcare setting.
- Master's degree preferred.
- Knowledge and experience with maintaining and securing medical records, including HIPAA and Quality Assurance reviews.
- Strong customer service orientation and experience resolving client and family questions or concerns.
- Experience maintaining accurate reports related to census, time and attendance, etc.
- Comfort with computer software applications, including Outlook, Word, and Excel.
- Excellent verbal and documentation skills.
- Ability to participate in Treatment Planning and perform needs assessments.
- Excellent organizational skills and ability to navigate the legal system and FMLA/Leave of Absence paperwork.
