Job Overview

Position Title: Community Living Specialist

Department: Community Living Services (CLS)

Reporting To: Community Living Advocate or Community Living Quality Coordinator

Supervisory Duties: Not applicable

Key Responsibilities:

  • Plan, prepare, and serve meals while encouraging hydration and monitoring dietary intake.
  • Assist individuals with daily living activities including eating, bathing, grooming, dressing, and personal hygiene, while ensuring comfort and safety.
  • Support with household tasks such as cleaning, laundry, and maintaining a safe living environment.
  • Provide guidance in financial management and assist with personal shopping and community engagement.
  • Coordinate medical appointments and assist with communication regarding health needs.
  • Facilitate transportation to various locations including stores, events, and medical visits, while adhering to mileage tracking policies.
  • Monitor vital signs and provide basic first aid as necessary.
  • Assist with medication management and reminders for self-administration.
  • Report any changes in the individual's condition or environment to relevant parties.
  • Complete all necessary documentation related to individual care, including shift notes and health monitoring.
  • Ensure compliance with training and documentation requirements through the Relias system.
  • Perform additional tasks as assigned by the supervisor.

Essential Skills and Qualifications:

  • Strong verbal and written communication abilities.
  • Excellent interpersonal skills and conflict resolution capabilities.
  • Effective organizational and time management skills, with the ability to work independently and prioritize tasks.
  • Proficient problem-solving skills.
  • Familiarity with Microsoft Office Suite and the ability to learn new software for documentation and communication.

Certifications: Preferred certifications include Certified Nursing Assistant or Patient Care Assistant. First Aid, CPR, and CPI (Nonviolent Crisis Prevention Intervention) certifications are required.

Education and Experience:

High school diploma or equivalent required.

Minimum of two years of relevant experience working with individuals with developmental and intellectual disabilities is required.
