We are seeking an exceptional Medical Social Work Professional to join our team at MultiCare Health System. As a key member of our healthcare team, you will play a vital role in delivering compassionate care and ensuring the well-being of our patients.

About the Role

This is an exciting opportunity for a skilled and dedicated individual to contribute to our mission of partnering for healing and a healthy future. The Hospital Care Coordinator will be responsible for cross-continuum care coordination, psychosocial assessments, and interventions that promote positive patient outcomes.

Key Responsibilities
  • Provide customer service to patients, families, physicians, and care teams to ensure seamless transitions of care.
  • Model and orchestrate consistent, professional communications with colleagues and staff.
  • Delegate responsibilities appropriately while remaining accountable for outcomes.
  • Ensure patients and families experience medically appropriate transitions of care.
Requirements
  • Master's degree in Social Work with a healthcare-based practicum.
  • Licensed in the state of Washington as a Social Worker Associate Independent Clinical (LSWAIC) with a commitment to obtain LICSW within four years.
  • Hospital or equivalent experience preferred; experience prior to obtaining a Master's degree may be considered.
  • Health industry knowledge preferred.
  • Healthcare financial assistance knowledge preferred.

The estimated salary range for this position is $81,245 - $116,938 per year, based on factors such as skill set, level of experience, and certification(s) and/or education.
