RN MDS Coordinator

Trinity Health is seeking a skilled RN MDS Coordinator to join our team. As a key member of our nursing staff, you will play a vital role in ensuring the highest quality of care for our residents.
Job Summary:The RN MDS Coordinator will be responsible for conducting and coordinating the development and completion of the Resident assessment in accordance with the requirements of this state and the policies and goals of this community. This includes evaluating, developing, mentoring, coaching, counseling, and disciplining department staff, as well as participating in community surveys and developing plans of correction for department deficiencies.
Key Responsibilities:- Evaluate and develop the Resident assessment in accordance with current rules, regulations, and guidelines.
- Conduct and coordinate the interviewing of each Resident for the Resident's assessment.
- Develop preliminary and comprehensive assessments of the nursing needs of each resident.
- Coordinate and oversee the entire RAI process, including completion of documentation as necessary to ensure that time frames are met by the interdisciplinary team.
- Collaborate with case managers and other interdisciplinary members to coordinate plan of care with discharge plans.
- Ensure that the care plan includes measurable objectives and timetables to meet the Resident's medical, nursing, and mental and psychosocial needs as identified in the Resident's assessment.
- Manage the care coordination of all managed care patients and monitor the resource consumption using tools provided.
- Provide reports to Administrator/Executive Director, Director of Nursing, and other members of the interdisciplinary team to support areas needing attention.
- Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved RN program.
- Must possess a current, unencumbered, active license to practice as a Registered Nurse in this state.
- Must be AANAC Certified or obtain certification within 6 months of hire.
- Two to three (2-3) year(s) of experience related to the MDS process, preferred.
- Must possess and maintain current CPR Certification.
- Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
