Salary: $105,000 - $110,000 per year

Sign-on Bonus: $15,000 (tax-free)

Our client is a renowned long-term care organization dedicated to providing exceptional care to older adults. We are seeking a highly skilled and compassionate CHHA-Intake/Outreach Manager to join our team.

Job Summary:

The CHHA-Intake/Outreach Manager will be responsible for managing the screening, evaluation, and admission planning for our CHHA Program. This includes coordinating with internal and external vendors to ensure timely evaluation and admission. The successful candidate will have a minimum of two years of experience assessing patients for home care safety and developing appropriate care plans. A valid driver's license and current NYS RN License are required.

Responsibilities:

  • Manage the intake process from inquiry to admission acceptance
  • Coordinate with Nurse Care Managers and internal/external vendors for timely evaluation and admission
  • Establish new referral sources in collaboration with the Associate Administrator of Provider Relations
  • Perform direct services as a Nursing Care Manager (may be required)

Requirements:

  • Minimum two years of experience assessing patients for home care safety and developing care plans
  • Minimum one year of supervisory experience in hospital or home care agency
  • Patient Review Instrument (PRI) assessor skills experience preferred
  • Valid driver's license required
  • MS Office required
  • Current NYS RN License and registration
