Job Title: Registered Nurse - Home Health

Rebekah Certified Home Health Agency is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health RN. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to our patients in the comfort of their own homes.

Responsibilities:
  • Conduct thorough assessments of patients' physical and mental status, identifying their needs and developing individualized care plans.
  • Collaborate with patients, families, and healthcare professionals to ensure seamless care coordination and effective communication.
  • Provide skilled nursing services, including wound care, medication management, and education on disease management and self-care.
  • Supervise and train Home Health Aides and other healthcare professionals to ensure they have the necessary skills and knowledge to provide high-quality care.
  • Document patient progress, including assessments, care plans, and outcomes, in a timely and accurate manner.
  • Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to ensure compliance and excellence in patient care.
Requirements:
  • Current licensure as a Registered Professional Nurse in New York State.
  • Minimum 2 years of experience as a Registered Professional Nurse, with a focus on home health care or a related field.
  • Strong clinical skills, including assessment, planning, implementation, and evaluation of patient care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Ability to work independently and as part of a team, with a strong commitment to patient-centered care.
  • Valid NYS driver's license and reliable transportation to conduct home visits.
