About the Role:

We are seeking a Long-Term Care MDS Coordinator to join our team at Rochester Rehab & Living Center. In this role, you will be responsible for coordinating the MDS process, ensuring compliance with regulatory requirements, and collaborating with the IDT to provide quality care to our residents.

Key Responsibilities:

  • Schedules and codes new admissions and assessments with the RAI/PPS Coordinator
  • Completes MDS and CAAS within required time frames
  • Collaborates with IDT members to determine resident significant change in status and OMRA assessments
  • Ensures completion of nursing sections of MDS per facility policy and updates facility MDS schedule and tracking forms

Requirements:

  • Current licensure as a Registered Nurse in the state of employment
  • Previous long-term care experience with knowledge of the RAI process preferred
  • Able to display knowledge of RAI/PPS process and follow all infection prevention and control requirements
