We are seeking a skilled Care Manager to join our team at Emory Healthcare. As a key member of our healthcare team, you will be responsible for coordinating the care of complex patients across the continuum, ensuring high-quality clinical and cost outcomes.

Key Responsibilities
  • Communicate effectively with hospital staff, physicians, and payors to ensure seamless care coordination
  • Convey a positive impression of the organization through excellent communication and interpersonal skills
  • Reinforce patient education and ensure it is carried out according to plan across the continuum
  • Provide ongoing educational opportunities to enhance healthcare teams' knowledge of case management services
  • Participate in and facilitate patient/family education and communication
Requirements
  • Minimum of three years varied hospital experience or comparable job-related experience
  • Three to five years of recent acute hospital experience, three to five years of discharge planning experience, preferred
  • Graduate of an accredited school of nursing. ACM or CCM certification required within one year of eligibility
  • BS in Nursing or health-related field hospital/case management/discharge planning, preferred
  • All nurses employed in roles requiring an unencumbered nursing license must have a valid, active license or temporary permit approved by the Georgia Licensing Board
  • BLS certification required
