About the Job

We are seeking an experienced Nurse Practitioner to join our team. The ideal candidate will have a strong background in palliative care and be licensed to practice in the state where care is being provided.

Responsibilities
  • Provide comprehensive medical care to patients with complex needs
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Conduct regular assessments to ensure optimal patient outcomes
  • Communicate effectively with patients, families, and healthcare providers
Requirements
  • Graduate from an accredited school of nursing with a Master's degree in Nursing
  • One year of experience as a Nurse Practitioner preferred
  • Licensed to practice as a Nurse Practitioner in the state of agency operation
  • Active DEA license in the state where care is being provided
