Job Summary

Elderwood IPA seeks a skilled Outreach Specialist to promote our organization, drive awareness of partnering relationships, and identify membership growth opportunities in the WNY territory.

Key Responsibilities
  • Provide external outreach on behalf of Elderwood IPA to establish and maintain key relationships within designated territory to generate potential enrollment opportunities for high-quality referrals from relevant organizations.
  • Attend networking or other events to build relationships and generate new enrollment opportunities.
  • Regularly follow up with potential contacts and established relationships to discuss services provided by Elderwood IPA and partnering organizations.
  • Keep a list updated based on changes of contacts and introduce new contacts.
  • Coordinate with some provider outreach teams to share related events and other outreach activities.
  • Interact with interested potential enrollees to answer product and/or service questions at communities.
  • Assess needs with a basic screen of candidates to identify qualified, eligible potentials related to both long-term care needs and Medicaid coverage for enrollment.
  • Address questions and concerns, effectively and clearly present benefits of products in accordance with all Governmental regulations.
  • Complete pre-enrollment screening forms for all potential enrollees.
  • Provide linkage for potential enrollee information to partnering organizations for further follow-up and qualification.
  • Complete all necessary paperwork, documentation, and follow-up as required.
Requirements
  • Valid NYS Driver's License.
  • Bachelor's Degree in Social Work or related field and 3-5 years of related experience OR Associates Degree with 7 years of related experience.
  • Bilingual fluency in Spanish, highly preferred.
  • Proficiency in Microsoft Office: Excel, Word, Access; Internet navigation, required.

  • Presentation Skills.
  • Excellent Communication.
  • Ability to solve diverse problems that require interpretation of administrative and/or technical issues.
