Job Title: Wellness Nurse

At Sunrise Senior Living, we are committed to providing high-quality care to our residents. As a Wellness Nurse, you will play a vital role in ensuring the overall well-being of our residents.

Responsibilities:
  • Identify and Meet Resident Needs: Work with our team to identify the ongoing needs and services of our residents, promoting the highest quality resident services.
  • Assess and Document Resident Care: Ensure proper follow-through, assessment, and documentation for residents with changes in healthcare services and status.
  • Conduct Resident Assessments: Assist the Resident Care Director in conducting resident assessments at move-in, 30 days post-move-in, semi-annually, and with any change in status.
  • Monthly Wellness Visits: Conduct monthly wellness visits for all residents, assessing, monitoring, and following up on any falls, accidents, or incidents.
  • Communicate with Families: Maintain open communication with residents' families and responsible parties to ensure medical needs are being met.
  • Collaborate with Healthcare Professionals: Work with attending physicians and other healthcare professionals as needed to ensure residents receive the best possible care.
  • Maintain Accurate Records: Document all pertinent information in resident wellness files, ensuring compliance with Sunrise policies and federal and state regulations.
Qualifications:
  • Education and Licensure: Graduate of an approved college or school of nursing, with a current state license as a professional Registered Nurse (RN).
  • Experience: Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Skills and Abilities: Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, as well as federal, state, and local regulations. Ability to handle multiple priorities, possess written and verbal skills for effective communication, and demonstrate good judgment, problem-solving, and decision-making skills.
