Job Summary

We are seeking a highly skilled and compassionate Physical Therapist to join our team in providing personalized home health services to patients in Wharton, North Wallace, Eagle Lake, and East Bernard areas.

Salary and Benefits

The successful candidate will receive a competitive salary range of $90,000-$105,000 per year, based on years of experience, along with comprehensive benefits, including medical, dental, and vision coverage, paid time off, professional development opportunities, company-matching 401(k), flexible spending accounts, and more.

Responsibilities

As a Home Health Physical Therapist, you will be responsible for:

  • Providing individualized physical therapy services to patients in their homes, focusing on rehabilitation and recovery.
  • Conducting thorough assessments, developing personalized treatment plans, and implementing effective interventions to promote optimal patient outcomes.
  • Collaborating with interdisciplinary teams to ensure seamless coordination of care and achieve shared goals.
  • Maintaining accurate records, documenting progress, and communicating effectively with patients, families, and healthcare professionals.

Requirements

To be considered for this role, you must possess:

  • A Master's degree in Physical Therapy from a CAPTE-accredited program.
  • At least nine months of experience as a licensed physical therapist.
  • Current licensure to practice as a physical therapist within the state of operation.
  • Valid CPR certification and a reliable means of transportation for travel to business locations.
