Job Summary

We are looking for a licensed Physical Therapist with a passion for home health care to work with us. As a Home Health Physical Therapist, you will have the opportunity to make a real difference in the lives of our patients, working one-on-one to develop and implement personalized treatment plans that help them achieve their goals and regain their independence.

Key Responsibilities:
  • Conduct thorough assessments of patients' physical abilities and create individualized treatment plans
  • Develop and implement effective therapy programs to help patients achieve optimal outcomes
  • Work collaboratively with patients, family members, and other healthcare professionals to ensure seamless care coordination
Requirements:
  • Master's degree in Physical Therapy from an accredited program
  • Current licensure to practice as a Physical Therapist in the state of operation
  • Minimum 9 months of experience as a Physical Therapist
  • PREFERRED: Community or home health experience
