Position Overview

We are seeking an experienced Licensed Practical Nurse (LPN) to join our Advanced Home Health team. As a LPN, you will coordinate the home care of chronically ill patients by utilizing a disease management process, the OASIS assessment, plan of care, and established protocols.

You will educate the patient/caregiver in the management of their illness and promote the home health support systems to decrease re-hospitalization and utilization of hospital emergency services.

Responsibilities
  • Coordinate home care for chronically ill patients
  • Utilize disease management process, OASIS assessment, plan of care, and established protocols
  • Educate patient/caregiver in illness management and promote home health support systems
Qualifications
  • Licensed LPN with current PA state license
  • Home Health experience preferred
  • Ability to work independently and within a multidisciplinary team
  • Valid driver's license necessary
