Job Summary

Dean's Professional Services is seeking a qualified Home Health Licensed Vocational Nurse to join our team in the Houston, TX area.

The successful candidate will be responsible for monitoring assigned cases to ensure compliance with third-party payor requirements, while also addressing patients' concerns and providing effective communication to patients, families, and healthcare professionals.

Key Responsibilities:

  • Participate in planning and implementing care in conjunction with the RN, by the Plan of Care, including wound care, medications, and upcoming vaccinations.
  • Assist in identifying patients' physical, psycho-social, and environmental needs, as evidenced by documentation, clinical records, case conferences, team reports, and evaluations.
  • Provide effective communication to patients, families, and healthcare professionals, as evidenced by clinical notes, case conferences, and communication notes.

Requirements:

  • High School Diploma – Required
  • Graduate of an accredited school of vocational nursing – Required
  • Must have Home Health Experience
  • Current Texas State license as a Licensed Vocational Nurse – Required
  • 2+ years' experience as a Licensed Vocational Nurse in a clinical care setting; home health – Preferred
  • Nursing skills as defined by generally accepted standards of practice; must have good interpersonal skills
  • Reliable transportation; valid and current Texas driver's license and valid and current auto insurance
  • Works in a patient's home under various conditions, PRN emergency calls
  • Possible exposure to bodily fluids and infectious diseases
  • Must have the ability to work a flexible schedule and to travel the agency's service area
  • There may be some exposure to inclement weather conditions
