Job Description:

About the Role:

The successful candidate will be responsible for providing comprehensive case management services, including assessment, planning, implementation, and evaluation. You will work closely with patients, families, healthcare providers, and community agencies to ensure seamless care coordination.

Responsibilities:

  • Assess patient needs and develop personalized care plans
  • Coordinate access to medical, mental health, and social services
  • Collaborate with patients, families, and healthcare providers to ensure effective care transitions
  • Develop and implement educational programs to promote healthy behaviors and disease management
  • Maintain accurate and timely documentation of patient interactions and care plans

Requirements:

  • Bachelor's degree in Social Work, Psychology, or related field required; Associate's degree with relevant experience may be considered
  • 2+ years of experience in case management, social work, or a related field
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Proficiency in Microsoft Office and electronic health record systems

Estimated Salary:

$32,000 - $40,000 per year
