Job Description
Clinical Nursing Coordination Duties
We are seeking an experienced Nurse Care Manager to join our team in Smyrna, GA. As the successful candidate, you will perform various tasks to assess and assist patients with chronic medical conditions. These responsibilities include:
  • Conducting comprehensive health assessments to identify areas of improvement.
  • Developing and implementing personalized care plans to promote desired outcomes.
  • Providing education and guidance to patients and their support systems.
  • Collaborating with healthcare providers and other stakeholders to ensure seamless care coordination.

Required Skills and Qualifications
To be successful in this role, you must have a Bachelor's degree in Nursing, 2+ years of clinical nursing experience, and a LPN or RN License. You should possess excellent communication and interpersonal skills, as well as a passion for delivering high-quality patient care.
