Clinical Care Coordinator

We are seeking a highly skilled Clinical Care Coordinator to join our team at Atlanta Fibroid Center. As a Clinical Care Coordinator, you will be responsible for coordinating care activities to ensure efficient delivery of quality patient care.
**Key Responsibilities:**
- Conduct ongoing nursing assessment as dictated by the patient's condition and in accordance with client facility protocols.
- Collaborate with other team members to maintain accurate and comprehensive patient records.
- Develop and implement individualized plans of care to meet patient needs and promote optimal outcomes.
Qualifications:
• Current Georgia Registered Nurse/RN or LPN License
• Advanced Cardiovascular Life Support (ACLS) certification from either an accredited instructor from the American Heart Association/AHA or the American Red Cross
• Minimum of one year paid Registered Nurse/RN or LPN work experience within the past year of direct patient care in a hospital
**Estimated Salary:** $90,000 - $120,000 per year
