Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Avamere Rehabilitation of Park West. As an LPN, you will play a vital role in providing high-quality patient care and contributing to a positive and supportive work environment.
Responsibilities:- Administer medications and review medication cards for completeness and accuracy.
- Coordinate with nursing staff and support personnel to plan daily services, programs, and activities.
- Develop and maintain rapport with nursing staff, residents, family members, and other individuals involved in patient care.
- Participate in facility surveys and maintain confidentiality of patient care information.
- Current, unencumbered, active license to practice as an LPN in this state.
- Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
- Experience with Electronic Medical Records and computer documentation systems.
