Job Title: Qualified Mental Health Professional (QMHP) Case Manager

At Carolina Therapeutic Services, we are seeking a highly motivated and compassionate Qualified Mental Health Professional (QMHP) Case Manager to join our team. As a QMHP Case Manager, you will play a vital role in providing holistic and supportive services to our clients, ensuring their continuity of care and achieving their treatment goals.

Responsibilities:
  • Coordinate behavioral health services and interventions with other professionals and stakeholders.
  • Engage community and natural supports in the person-centered planning process.
  • Deliver direct care services when necessary to ensure consumer continuity of care.
  • Monitor and document the status of the recipient's progress and the effectiveness of the strategies and interventions outlined in the Treatment Plan.
  • Collect data and submit for continuing service authorization, maintaining up-to-date service authorization tracker to sustain 100% authorizations on all consumers within the team.
  • Input and upload consumer demographics onto documentation site (OnTarget).
  • Consult with identified medical and non-medical providers to ensure comprehensive care.
  • Work collaboratively with the team to ensure agency compliance and achieve treatment goals.
Supervisory Responsibilities:
  • Monitor service delivery to ensure goals are achieved to meet consumer needs and ensure continuity of care.
  • Conduct weekly service follow-up/verification calls to consumers/guardian.
  • Conduct monthly outcome summaries and progress of consumer goals. Complete discharge summaries within 7-days of discharge.
  • Ensure services are provided in a manner that meets all applicable regulatory licensure and accrediting regulation.
  • Provide weekly supervisions to assigned staff, must be documented.
  • Review/Approve all assigned staff's service notes to ensure they are billable and in accordance with regulatory standards.
  • Create/Review and assign service schedules for assigned staff.
  • Provide routine monitoring for services being rendered by team members for quality assurance in accordance with Medicaid Part 132 requirements.
Requirements:
  • Master's Degree in a human service field.
  • Mental & Behavioral Health experience: 1 year.
  • Supervising: 1 year.
  • Spanish-speaking, preferred.
