About the Role

As a Physical Therapist, you will be responsible for providing skilled physical therapy services to patients in their homes. This includes evaluating patients, developing and implementing individualized plans of care, and providing ongoing therapy services to help patients achieve their goals.

You will work closely with our interdisciplinary team to ensure that patients receive comprehensive care that addresses their physical, emotional, and social needs.

Responsibilities

  • Communicate with patients, families, and healthcare providers to ensure seamless care
  • Document patient progress and outcomes
Requirements

To be successful in this role, you will need:

  • A current license to practice as a Physical Therapist in the state of employment
  • At least one year of experience as a Physical Therapist in an acute setting
  • Home care experience is desired
  • Ability to effectively communicate with patients, families, and healthcare providers
  • Knowledge of therapy best practices
