In this role, you will oversee the operations of the Medication Clinic, acting as a case manager for clients utilizing our services. Your responsibilities will include evaluating physical health requirements, making suitable referrals to local healthcare providers, and managing medication administration in collaboration with the psychiatrist. You will also deliver a comprehensive range of treatment, rehabilitation, and support services for individuals engaged in Assertive Community Treatment.
This is a part-time position requiring 20 hours of work per week, scheduled between 8:00 am and 5:00 pm on select weekdays, Monday through Friday.
KEY RESPONSIBILITIES
- Organize and coordinate the Medication Clinic operations.
- Review medication education materials with clients.
- Serve as a case manager for clients requiring medication refills.
- Complete necessary documentation related to the Medication Clinic and psychiatric consultations for client records.
- Administer intramuscular medications, dispense sample medications, and maintain accurate logs of all medications received and dispensed.
- Assist in completing prior authorizations.
- Maintain an updated list of injections, coordinate ordering and delivery, verify orders, and discuss any side effects with clients.
- Measure vital signs and facilitate the prediabetes screening process.
- Possess a broad knowledge and experience with computer systems.
- Under the guidance of the team psychiatrist, manage the medication delivery system for clients, ensuring integration of medication administration into individual treatment plans. Take a lead role or participate in providing medication administration and medical services.
- Provide education, training, and coaching on medications, mental health conditions, and co-occurring health issues to clients and the ACT team.
- Participate in daily staff meetings and treatment planning reviews, as well as weekly supervision sessions.
- Complete Authorizations of Disclosure.
- Consult with the ACT team regarding client health and medication concerns.
- Ensure proper storage and administration of medications (including injections) in compliance with Medication Dispensation, Sample, and Patient Assistance Program Policies.
- Adhere to standard precautions, including the use of protective barriers and proper disposal of infectious waste.
- Follow CLIA and OSHA regulations.
- Perform other duties as assigned.
- High School diploma or GED is required.
- Practical Nursing certificate is mandatory.
EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
Experience in either inpatient or outpatient psychiatric settings is preferred.
LICENSURE/CERTIFICATION
Must hold an LPN or GPN and pass the MO NCLEX exam within 90 days of graduation.
