Overview

Amedisys Inc. is seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Case Manager in Home Health. As a key member of our care team, you will be responsible for providing patient-centered care and collaborating with healthcare professionals to develop and implement individualized care plans.

Responsibilities
  • Perform patient assessments and develop comprehensive care plans in collaboration with the care team.
  • Make referrals to other disciplines as indicated by patient needs or document rationale for not doing so.
  • Teach patients and their families self-care techniques and promote patient health and independence.
  • Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
  • Complete timely and accurate documentation and communicate patient progress to the clinical manager and care team.
  • Plan and provide staff education and participate in clinical development and continuing education programs.
Qualifications
  • One (1+) year of clinical experience as a Registered Nurse (RN).
  • Current RN license, specific to the state(s) assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
