Position Overview

Home Health Experience Required

Adoration Home Health is actively seeking a PRN, Licensed Practical Nurse to assist our skilled nursing patients in the Memphis region. As a LPN at Adoration Home Health, you will play a vital role in our healthcare team, providing high-quality and compassionate care to patients in their residences. Key Responsibilities
  • Delivering one-on-one care for clients in accordance with their care plans while preserving their dignity and independence
  • Monitoring clients' vital signs and reporting any abnormal findings to the Case Manager and Physician
  • Administering medications, feedings, oxygen, and ostomy care as required by the client's care plan and assessing outcomes
  • Completing routines established by other healthcare professionals such as Physical Therapists, Occupational Therapists, and Speech Therapists
  • Ensuring compliance with infection control policies, reporting potential communicable diseases or infections, and implementing infection control procedures
  • Accurately documenting all nursing actions, communications with family, case managers, and physicians, as well as tracking progress and outcomes for established goals
  • Coordinating or ordering necessary supplies, medications, and feedings to ensure adequate availability at all times
  • Ensuring all patient needs are met in accordance with physician instructions
  • Monitoring incidents involving individuals, ensuring timely and accurate completion of incident reports, and informing case managers while examining individuals post-incident
  • Maintaining all medical equipment and reporting any faulty equipment to DME for repair
  • Performing other duties as assigned
Qualifications
  • LPN/LPT/LVN Degree from an accredited nursing college with current unrestricted registration and license in the state of practice
  • Valid driver's license, acceptable driving record, and proof of car insurance
  • CPR professional certification
  • Experience in medication administration
  • Preferred 1+ years of nursing experience
  • Preferred experience in home health care
  • Able to lift and carry items weighing up to 20 pounds
  • Able to bend, stoop, push, pull, sit, and walk for extended periods
  • Must possess effective verbal and written communication skills
