Position Summary
We are currently seeking a PRN, Licensed Practical Nurse (LPN) to provide exceptional care to our skilled nursing patients. As a vital member of our healthcare team, you will play a crucial role in delivering compassionate and high-quality care to patients in their residences.
Key Responsibilities
- Deliver personalized 1:1 care, adhering to each client's care plan while respecting their dignity and independence.
- Monitor vital signs and report any irregularities to the Case Manager and Physician.
- Administer medications, feedings, and other necessary treatments as outlined in the client's care plan.
- Collaborate with other healthcare professionals, including Physical Therapists and Occupational Therapists, to ensure comprehensive care.
- Maintain strict infection control protocols and report any potential health risks.
- Document all nursing actions and communications with family members, case managers, and physicians accurately.
- Coordinate the procurement of medical supplies and medications to ensure availability.
- Address all patient needs in accordance with physician directives.
- Monitor incidents involving patients and ensure timely and accurate reporting.
- Ensure proper maintenance of medical equipment and report any issues for repair.
- Perform additional duties as assigned.
Qualifications
- Current and unrestricted LPN/LPT/LVN license from an accredited nursing program.
- Valid driver's license with a satisfactory driving record and proof of insurance.
- CPR certification.
- Experience in medication administration.
- Preferred: 1+ years of nursing experience, particularly in home health care.
- Ability to lift and carry items weighing up to 20 pounds.
- Capable of bending, stooping, pushing, pulling, sitting, and walking for extended periods.
- Effective verbal and written communication skills.
