Job Summary

We are seeking a skilled Clinical Liaison to join our team at Providence Service. As a Clinical Liaison, you will play a critical role in creating and maintaining positive customer relationships to increase referrals for our services.

Key Responsibilities
  • Develop and maintain strong relationships with discharge planners, facility staff, physicians, and individuals to assess appropriateness for our services.
  • Facilitate the coordination of services and ensure proper documentation for compliance with agency and statutory guidelines.
  • Work with our team to ensure a smooth transition of patients from acute care facilities to their homes or next location of residence.
Requirements
  • Education sufficient for clinical licensure for discipline (e.g., LPN, LVN, Registered Nurse) or Bachelor's Degree in Social Work.
  • Current state professional license or certification appropriate to discipline upon hire.
  • 3 years experience in clinical discipline.
Preferred Qualifications
  • Master's Degree in Social Work.
  • Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy).
  • Liaison, marketing or sales experience.
  • Healthcare industry experience.
  • Documented prior successful sales account medical management experience.
  • Epic EMR experience.
