We are seeking a skilled Clinical Liaison to join our team at Providence Health & Services. As a Clinical Liaison, you will play a critical role in creating and maintaining positive customer relationships to increase referrals for our services.
Key Responsibilities- Develop and maintain strong relationships with discharge planners, facility staff, physicians, and individuals to assess appropriateness for our services.
- Facilitate the coordination of services and ensure proper documentation for compliance with agency and statutory guidelines.
- Work collaboratively with our team to ensure seamless transitions of patients from acute care facilities to their homes or next locations of residence.
- Education sufficient for clinical licensure or a Bachelor's Degree in Social Work.
- Current state professional license or certification appropriate to discipline upon hire.
- 3 years of experience in a clinical discipline.
- Master's Degree in Nursing or a related field.
- Liaison, marketing, or sales experience.
- Healthcare industry experience.
