Job Summary

The Clinical Liaison position is a key role in creating and maintaining positive customer relationships to increase referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit, and other services within assigned accounts.

Key Responsibilities
  • Work with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services.
  • Facilitate the coordination of services and ensure there is appropriate documentation for compliance with agency and statutory guidelines.
  • Manage the transition of patients from acute care facilities to their homes or next locations of residence.
Requirements
  • Education sufficient for clinical licensure for discipline (e.g., LPN, LVN, Registered Dietitian) or Bachelor's Degree in Social Work.
  • Current state professional license or certification appropriate to discipline upon hire.
  • 3 years of experience in clinical discipline.
Preferred Qualifications
  • Master's Degree in Nursing.
  • Bachelor's Degree in Applicable clinical discipline (e.g., Respiratory Therapy).
  • Liaison, marketing, or sales experience.
  • Healthcare industry experience.
  • Documented prior successful sales account medical management experience.
  • Epic EMR experience.
