Key Responsibilities:

The successful candidate will be responsible for executing a sales strategy to increase market share through account development and educating the medical community about our services. Additionally, they will facilitate seamless transitions for patients moving from facilities to home care environments.

  • Assist patients in navigating post-acute care needs after selecting NHC HomeCare as their healthcare provider.
  • Assess, plan, implement, coordinate, monitor, and evaluate options and services to provide a safe transition from facilities to home for home health.
  • Ensure patients have a physician and obtain an order for their home health plan of care.
  • Obtain Face-to-Face documentation and communicate it to the appropriate agency.
