Key Responsibilities
  • Assessment and Goal Setting: Conduct thorough assessments of patients and their families' biopsychosocial situations, establishing clear goals and periodically reassessing and modifying them as needed.
  • Crisis Intervention and Support Services: Provide crisis intervention services for cases involving domestic violence, psychiatric emergencies, child abuse/neglect, family disputes, end-of-life care, bereavement, and Social Determinants of Health. Assess and help address issues impacting compliance with treatment plans.
  • Collaboration and Relationship Building: Work closely with the multidisciplinary team to address patients' mental and behavioral health needs, establishing and maintaining effective working relationships across various disciplines.
  • Community Resource Assistance: Assist patients in accessing community resources and support services, serving as a liaison between the clinic and external agencies, providing referrals as appropriate.
  • Resource Awareness: Stay informed about available community resources, providing families with specific information related to these resources.
  • Case Conferences and Documentation: Participate in case conferences and team meetings to discuss patient progress and treatment plans, maintaining accurate and up-to-date documentation in the Electronic Medical Record system, adhering to Medicaid, CMS, and other regulatory requirements.
Requirements
  • Masters degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)
