We are seeking a skilled Registered Nurse to join our team as a Dialysis Nurse Specialist. In this role, you will be responsible for providing high-quality patient care and education in a skilled nursing facility (SNF) setting.
Key Responsibilities- Provide comprehensive patient care, including dialysis treatment and education
- Collaborate with SNF staff to coordinate care and ensure seamless transitions
- Develop and implement individualized care plans to meet patient needs
- Conduct patient education and training on dialysis treatment and self-care
- Supervise dialysis technicians and ensure they are meeting quality and safety standards
- Current Registered Nurse (RN) license in the state of practice
- Minimum of 12 months' of RN experience
- Previous experience in chronic hemodialysis, acute dialysis, and/or peritoneal dialysis nursing
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing
