Key Responsibilities- Conduct resident assessments and develop personalized social care plans
- Collaborate with residents, families, and healthcare professionals to identify and address social and emotional needs
- Provide information and support to residents and families regarding Medicare, Medicaid, and other financial assistance programs
- Develop and implement social service programs and activities to enhance resident quality of life
- Maintain accurate and informative charted progress notes
- Participate in quality assurance and quality improvement initiatives
- At least one year of experience in social work, preferably in a long-term care facility
- Effective verbal and written English communication skills
