POSITION SUMMARY:

Homewell Care Services is dedicated to providing exceptional in-home support within the Glendale community and its neighboring areas. We are currently seeking committed and skilled individuals to fulfill the role of a caregiver.

As a caregiver, you will assist clients with their daily living activities as specified in a personalized care plan created by the agency's Care Manager. This role encompasses both direct personal care services and supportive tasks that may include light housekeeping, meal preparation, shopping, errands, transportation, and companionship.

This position is considered safety-sensitive.

KEY RESPONSIBILITIES:

  • Comprehend and effectively execute a client's care plan.
  • Provide assistance to clients as outlined in their care plan, which may involve companionship, meal preparation, light housekeeping, transportation, shopping, medication reminders, bathing, personal hygiene, dressing, feeding, mobility support, positioning, transferring, toileting, and supervision.
  • Report any client concerns, changes in care, or emergencies to the Care Manager promptly.
  • Inform the Care Manager and/or Staffing Coordinator of any safety issues.
  • Adhere to company Infection Control protocols and notify the office if Personal Protective Equipment (PPE) or supplies are needed.
  • Utilize ClearCare Telephony Instructions for clocking in and out for scheduled shifts, ensuring accurate task status updates for each client before leaving their residence. For any compensable hours not assigned to a specific client (such as training), submit the Time Log and Time Certification for Non-Visit Time form for each pay period.
  • Follow the company dress code as outlined in the Employee Handbook.
  • Arrive at work punctually and prepared for the day.
  • Engage in ongoing training as required by client care needs and/or changes in company policies.
  • Participate in staff meetings.
  • Comply with all company policies and procedures as detailed in the Employee Handbook.

SUPERVISORY RELATIONSHIP: The caregiver operates under the supervision of the Care Manager and receives scheduling direction from the Staffing Coordinator.

PHYSICAL AND ENVIRONMENTAL REQUIREMENTS: Capable of repositioning and transferring clients, managing the weight of clients using appropriate transfer safety techniques and equipment (e.g., transfer belt), and traveling to clients' residences using reliable transportation.
