RN Care Coordinator

Key Responsibilities:- Assess, plan, and implement care strategies that prioritize the most appropriate, least restrictive level of care for each member.
- Identify and initiate referrals for member transitions to community resources and provide assistance with social service programs, including financial, psychosocial, community, and state supportive services.
- Manage the care plan throughout the continuum of care as a single point of contact, communicating with all stakeholders to ensure quality coordinated care and services are provided expeditiously.
- Advocate for members and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.

- A current, unrestricted RN license in the state of Texas.
- 2+ years of experience working within the community health setting or in a healthcare role.
- Intermediate level of computer proficiency, including Microsoft Outlook and Teams, and the ability to use multiple web applications.
- Reliable transportation and the ability to travel in the assigned region to visit Medicaid members in their homes and/or other settings.
- Access to a designated quiet workspace in your home, with the ability to secure Protected Health Information (PHI).
- Reside in a location that can receive a UnitedHealth Group-approved high-speed internet connection or leverage an existing high-speed internet service.
- 1+ years of experience with long-term care services and support, Medicaid, or Medicare.
- Ability to create, edit, save, and send documents, spreadsheets, and emails.
- Knowledge of the principles of most integrated settings, including federal and state requirements.
