Job Summary

We are seeking a highly skilled Clinical Care Navigator to join our team at CommonSpirit Health. As a Clinical Care Navigator, you will play a key role in coordinating patient care across the continuum, promoting optimal clinical, quality, and cost-effective outcomes, and advocating for patients' needs.

About the Role

The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work collaboratively with multidisciplinary teams. They will be responsible for developing and implementing individualized care plans, maintaining accurate documentation, and participating in process improvement activities.

Requirements
  • Graduation from an accredited school of nursing plus three years of recent clinical work experience in the specialty area or related clinical specialty in a hospital setting within the past five years.
  • Active Washington State license as a Registered Nurse is required.
Salary and Benefits

The estimated salary for this position is $95,000 - $130,000 per year, depending on experience. We also offer a comprehensive benefits package, including health/dental/vision, FSA, matching retirement plans, paid vacation, adoption assistance, annual bonus eligibility, and more.
