As a key member of our interdisciplinary team, you'll be responsible for accurately assessing residents' health status, educational, and environmental needs. You'll communicate significant changes in condition to the appropriate team members and document all assessments.

Your dynamic care plans will be developed through a collaborative approach with physicians, residents, families, and other team members. You'll review and revise the plan as the resident's needs, problems, and goals change.

Key responsibilities include:

* Accurately documenting all assessments and care plans
* Developing and implementing individualized care plans
* Communicating effectively with residents, families, and team members
* Maintaining composure in crisis situations
* Identifying problem areas and using the QAPI process to address them

To succeed in this role, you'll need:

* A degree from an accredited school of nursing
* Current RN license in the state you'll be working
* CPR/AED certification or willingness to obtain within six months
* Long-term care experience preferred
* Critical thinking skills
* Excellent communication and time management skills
* Ability to stand and walk for long periods
* Ability to lift heavy objects and transfer patients safely
