Job Title: Travel Rehab RN

Evolve Healthcare is seeking a skilled Travel Rehab RN to join our team for a travel contract in an inpatient acute rehabilitation hospital in Edwardsville, IL.

Job Summary:

We are looking for a dedicated and compassionate Travel Rehab RN to provide high-quality patient care and support our interdisciplinary team. As a Travel Rehab RN, you will be responsible for planning and delivering direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.

Responsibilities:
  • Accurately perform patient assessments and identify patient needs.
  • Identify and initiate appropriate nursing interventions.
  • Provide patient and family teaching throughout the rehabilitation stay and prior to discharge.
  • Provide care appropriate to condition and age of the patient.
  • Work cooperatively and actively participate with other members of the interdisciplinary team.
  • Participate in Continuous Quality and Performance Improvement (CQPI).
  • Maintain positive relationships with all customers.
  • Promote rehabilitation nursing philosophy and scope of practice.
  • Actively participate in team conferences.
  • Perform timely and appropriate documentation relating to medical necessity in the medical record.
  • Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
  • Perform timely and accurate QI assessments.
  • Provide a safe environment for patients, visitors, and staff and comply with appropriate and approved safety standards.
  • Support the leadership for the hospital in a professional manner and act as a role model.
Requirements:
  • Must have experience working in a hospital or rehab unit.
  • Traveler must have completed 1-2 years of acute care or inpatient rehab experience.
  • Preferred if traveler has completed at least one 13-week contract.
  • EMR: Meditech Expanse.
  • Traveler must have an active state licensure as Registered Nurse.
  • BLS - must be certified through American Heart Association ONLY.
  • ACLS - must be certified through American Heart Association ONLY.
  • CRRN Preferred.
  • Minimum six months' Medical/Surgical experience in an acute care setting preferred.
  • Excellent oral and written communication and interpersonal skills.
Education:

Graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing, or Nursing Diploma program.
