Position Overview:
As a Clinical Coordinator, you will play a vital role in advancing our mission by delivering high-quality service through effective case management and counseling. This position is ideal for those with experience in supporting individuals with developmental disabilities or mental health challenges.
Key Responsibilities:
- Manage a designated caseload by providing comprehensive case management and counseling services. Conduct assessments for referred individuals and lead the development, coordination, and evaluation of service plans in collaboration with the treatment team, funding sources, and key stakeholders.
- Ensure that all records are maintained accurately and in compliance with corporate, contractual, and regulatory standards.
- Offer ongoing monitoring and consultation to mentors regarding the implementation of service plans, including pre-service introductions and continuous education.
- For caseloads that include mentor individuals, conduct home visits and facilitate mentor group meetings as necessary.
- Provide emergency coverage and crisis intervention support as required, including participation in a rotational on-call system.
- Assist with legal, licensing, regulatory, and certification activities as needed.
- Engage in activities that foster the development of a robust network of mentors.
Qualifications:
- Master's degree in human services; Bachelor's or Master's in Social Work if mandated by state regulations.
- A minimum of two years of relevant experience working with individuals facing developmental disabilities or mental health issues.
- Self-motivated, detail-oriented, and capable of managing multiple tasks effectively.
- Proven ability to inspire a team and drive results to achieve organizational goals.
- Valid driver's license in good standing and access to a registered vehicle with proof of insurance for transportation purposes.
- Appropriate licensing (LBSW, LMSW, LCSW, or LCSW-C) as required by state and/or agency regulations.
